2. Completely fill out the application (No additional pages or writing on the back will be considered. Please make sure it is legible.)
3. Include one (1) head shot and one (1) full body shot in a bathing suit - label each photo on back with your name and phone number staple to application
4. Send a $20 check or money order made out to "THE ULTIMATE BODY TV SHOW" (to cover administrative fees) ... Walk-ins on the days of the event will pay $30 application fee.

Application Requirements
Requirements: The casting is open to men and women ages 18 to 100 residing anywhere in the world. Anyone who has ever won (awarded 1st place) in a bodybuilding, physique, fitness or bikini competition is ineligible. You must be able to communicate in English to participate and be able to endure the rigorous fitness regiment to be in body building shape. You must be in excellent physical condition and want to undergo the body transformation through a rigorous body building routine. The taping of the show will be in Florida, USA. You can reside anywhere in the world but will assume all costs to get to the show location and commit to the filming schedule if selected. If selected living expenses (i.e.; room, diet, nutrition program, trainers etc.) will be furnished free of cost and without hidden fees. If selected you will submit a recent exam from your MD verifying your health status. You must be willing to participate for 10-13 weeks in early 2010. If considered you will undergo a brief interview, in which you may be filmed, before final selection is made. Further details upon notification of consideration.
